Terms and conditions for treatment

• All patients will require a new patient assessment appointment.
• You will be asked to complete a medical form and detail any medications taken, allergies, and current and historical relevant medical conditions/history. Patients will not be treated without completion of the medical form and consent form for treatment.
• Any information provided is private and will not be shared with any other parties without patient consent.

Cancellations

• 24 hours’ notice is required for cancellation.
• Any appointment not cancelled within 24 hours must pay the full appointment/consultation fee Exemption due to exceptional circumstances may occur such as bereavement, exceptional illness or extreme mitigating circumstances but must be agreed by Coastal Podiatry.
• By booking an appointment you agree to your cancellations policy.
• Emergency appointments are available upon request, subject to availability.
